התפיסה המחקרית העדכנית רואה בפסיכוזה "רצף" (Continuum) ולא קטגוריה דיכוטומית, הנעה מחוויות תפיסתיות קלות ועד להפרעות מורכבות כמו סכיזופרניה. המחקר בתחום הנוירו-ביולוגי מדגיש את מודל ה"פגיעוּת-דחק" (Stress-Vulnerability Model) – השילוב שבין נטייה גנטית לבין גורמי סביבה וטראומה. מחקרים ארוכי טווח מוכיחים כי התערבות מוקדמת, במיוחד במהלך "התקופה הקריטית" של השנתיים הראשונות לאחר הופעת התסמינים, היא הגורם המנבא המשמעותי ביותר לשיפור בפרוגנוזה ובתפקוד החברתי. כיום, המחקר הקליני נותן דגש הולך וגבר לא רק להפחתת תסמינים חיוביים (כמו מחשבות שווא והזיות), אלא גם לשיפור התפקוד הקוגניטיבי והחברתי דרך שילוב של טיפול תרופתי מותאם יחד עם התערבויות פסיכו-סוציאליות וטיפול קוגניטיבי-התנהגותי המותאם לפסיכוזה.
